Summary
We are looking for a Support Worker to join our team at Age Concern Tyneside South (ACTS) working flexibly in the community for the Home From Hospital & Home Support services. This role involves undertaking a range of practical day to day tasks for individuals in their own homes and in the community. Helping people to overcome practical, social and emotional problems they face on returning home from hospital.
Also, to support vulnerable people to prevent a possible admission to hospital. Provide ongoing practical support to people in their own homes, carrying out a range of tasks & duties to enable them to remain as independent as they are able to be.
This role involves travel around the borough of South Tyneside, therefore it is a requirement of the role that you must have the ability to travel freely for work purposes.
Age Concern Tyneside South (ACTS) is a registered charity which promotes the wellbeing of older people living in South Tyneside, improving their quality of life and helping them maintain their independence.
