We are also recruiting 2 additional support workers for the Home from Hospital team .
16 hour contracts are available on both services.

Description:
To be a Support Worker for Age Concern Tyneside South (ACTS), working flexibily in the community for the Home from Hospital & Home Support services and in the ACTS office to support the needs of older people and the needs of the charity,
To deliver Home from Hospital & Home Support services unertaking a range of practical day to day tasks for individuals in their own home and in the community.
Help peopke overcome practical, social and emotional problems they face on returning home from hospital.
To support vulnerable people to prevent a possible admission to hospital.
Provide ongoing practical support to people in their own homes, carrying out a range of of tasks and duties to enable them to remain as independant as they are able to be.
Tasks to be agreed in advane by providing as assessment of the service user’s requirements and aims.
