This includes helping older people to be financially secure; to live in warm and safe homes; to have good social connections within their own neighbourhoods; to stay as physically healthy as they can and manage their health issues positively; to have choice and control over their lives; to have access to the right support for themselves as they need it; to access the correct support around mental health, including help accessing statutory, voluntary and practical support to maintain independence and specialist support on hospital discharge.
This includes access to the best available information so older people can decide what’s right for them; supporting people to have the confidence to ask the questions they need to ask, to ensure the options they want are available to them; helping them navigate complicated processes; and creating opportunities for peer support.
This includes working with partners, both operationally and strategically, to improve services and build neighbourhood capacity, to better support and empower older people, culminating in an Age Friendly South Tyneside.
